Protection and Use of Patient Information
Patient information and records will be kept in a secure and confidential manner and use will restricted to the terms of the Data Protection Act (1998).
We have an obligation to record relevant clinical details about our patients. This information is mainly recorded on a practice computer system. Presently these computer records are required to be kept indefinitely. They are available on a need to know basis to clinicians and administrative staff in the practice. The information in the patient records is used to facilitate ongoing care, for recall and for other administrative purposes such as repeat prescriptions. We do use clinical information for audit and research but this is in an anonymised form with no patient identifiable details.
We have strict protocols for access and confidentiality. Information from the clinical record may be released to third parties for your clinical care, for example when a referral is made to a hospital or community nurses. In some cases for example where NHS organisations use the same System One records system, your records will be shared and available to help ensure consistency of care and your safety. In these examples consent for release of or access to relevant clinical details is assumed. On other occasions such as for a report to a solicitor or insurance company we will ask for your signed consent. We do not release clinical details to other third parties without explicit consent from the patient. We cannot discuss patient's details with friends or relatives without the patient's consent.
As a patient you are entitled to view your clinical record and amend inaccuracies if found. There is normally an administrative charge for this service.
Freedom of Information Act - We will comply with the requirements of the Freedom of Information Act and a release plan will be available when our surgeries open.
