Registration
If you live within our catchment area as shown on the Surgery Locations page you may register with us as a patient. You will be allocated a main surgery according to each site’s catchment area, but this will not affect your right to attend any of the 3 surgeries operated by Haxby Group in Hull.

Due to the requirement for a signature, proof of identity and photo identification you will still have to come to one of our surgeries to actually register. When you do you will be asked to complete a short form giving some details and will be asked to attend an initial appointment where some basic health checks will be carried out by a nurse or GP. This will help to ensure that we provide the best possible healthcare for your needs.
