Registration
If you live within our catchment area as shown on the Surgery Locations page you may register with us as a patient. You will be allocated a main surgery according to each site’s catchment area, but this will not affect your right to attend any of the 3 surgeries operated by Haxby Group in Hull.
Our Kingswood List is currently closed. However, through our desire to maximise patient choice, you are still able to register at our either our Orchard Park or Priory Road Surgeries and we will still be happy for you to attend any of our thee Hull surgeries, including Kingswood.
Due to the requirement for a signature, proof of identity and photo identification you will still have to come to one of our surgeries to actually register. When you do you will be asked to complete a short form giving some details and will be asked to attend an initial appointment where some basic health checks will be carried out by a nurse or GP. This will help to ensure that we provide the best possible healthcare for your needs.
